    What is Care Coordination for Senior Care Plus Members?

    Cost-saving isn’t the only reason to enroll in a Medicare Advantage Plan. One of the main reasons Medicare beneficiaries in Nevada join a Senior Care Plus Medicare Advantage Plan is for the care coordination services. The Senior Care Plus Care Coordination team helps members navigate what can be a complex healthcare system. Care coordination is a popular and extremely important service for members because keeping members healthy is the number one goal. One way they help reach this goal is to encourage members to participate in a no-cost, comprehensive health assessment. At this Quick Start Health Assessment, members meet with a geriatric specialist – a provider who specializes in the care of seniors – to discuss the 4 Ms: Mentation – Thinking, memory and mental health Medications – Understanding your medication Mobility – Staying physically active What Matters to You – Let your provider know what is important to you – examples could be family, health and independence The results of this detailed visit are then shared with the member’s primary care provider, so a customized care plan can be developed. Care Team Approach – Laying the Foundation to Improve Health Health assessments and annual visits are offered so Renown Health providers can build relationships to improve care. This approach, also known as the Building Relationships to Improve Care or BRIC Model, is the care model used across Renown Health. “What’s special about this care model is that it really puts our patients at the center of their care,” says Savannah Gonsalves, a registered nurse with Senior Care Plus. “Members have their providers and nurses, Senior Care Plus personal assistants, case managers, and within the BRIC Model, they’re all talking to one another and putting the focus on the patient to meet needs.” Personal Assistants – A Unique Connection to Each Member A team of personal assistants is available to help members coordinate care by: Scheduling a member’s appointments Answering a member’s benefits questions Helping navigate care – these are experts in both health insurance and healthcare Answering questions about medications Working with providers to coordinate a member’s care The Senior Care Plus personal assistants are one of the most popular services that the Medicare Advantage plan offers.     Renown Health & Cigna Reach Agreement

    New contract provides Cigna members enrolled in employer-based HMO & PPO plans in-network access to Renown Medical Group and Renown hospitals  Renown Health leaders have reached agreement on a new contract with Cigna that will go into effect starting today, Nov. 1, 2022. Leaders were able to work collaboratively towards a positive solution that will help further both team’s collective goals of providing expanded access to quality, affordable healthcare in northern Nevada.     Dispatch Health Partners & Renown to Provide In-Home Medical Care

    Hospital network brings high-quality, affordable, on-demand acute care to local residents. The partnership aims to provide patients with another option for safe access to medical care during the COVID-19 pandemic and beyond. DispatchHealth and Renown’s goal is to offer the most vulnerable patients at-home care options if it is in the best interest of their health needs. DispatchHealth’s model is uniquely positioned to help treat an extensive range of common to complex injuries and illnesses, including but not limited to urinary tract infections, dehydration, fall injuries, nausea, sutures and more and is accepted by most major insurance plans. If a patient is uninsured, there is a flat fee of $275, which includes all medications administered and lab tests on site. “It’s more important than ever to be able to offer patients convenient, high-acuity medical care that is also cost-efficient,” said Bill Butcher, regional market director, DispatchHealth. “The partnership with Renown Health provides safe care for people without them having to leave their home, helping to ensure community members get the right care at the right place at the right time.” Patients requiring medical services for sudden or urgent illness, such as sore throat or skin rashes, can request care by going to the DispatchHealth website, accessing the free mobile app  or calling 775-439-1529. Once contacted, the medical team arrives within a few hours to treat the patient in their place of need and consists of either a physician assistant or nurse practitioner, along with a medical technician. DispatchHealth’s medical teams wear appropriate personal protective equipment (PPE) during all visits, including masks, gloves, and protective eyewear. Additionally, kits and devices are sanitized during and between visits. The medical team is able to perform a variety of advanced tests and treatments, ranging from blood tests to a 12-lead EKG, to IV fluids and more. To ensure continuity of care, DispatchHealth provides a detailed report to each primary care physician, living community, or a home health agency, in addition to electronically sending prescriptions to a patient’s pharmacy if needed. For emergencies, including but not limited to, chest pain, signs of a stroke, allergic reactions or severe injuries, individuals are urged to call 911 and seek safe, acute care in one of Renown’s emergency rooms.
