What is ear wax and why is it healthy? Ear wax is a protective substance called sebum made of fat, skin cells and sweat. Ear wax is produced by glands in the ear canal. It helps protect the ear from germs and dirt and prevents the sensitive skin of the ear canal from becoming irritated. It is normal and healthy to have a thin lining of ear wax on the canal. Ear wax will sometimes accumulate and move to the outer ear canal, where it would typically fall out or be wiped away. This is also normal. When should I seek care? (Nasdaq: GILD) and the Renown Institute for Health Innovation (IHI) today announced a strategic collaboration to collect and analyze genetic and electronic health data that can enhance the understanding of nonalcoholic steatohepatitis (NASH) and potentially inform development of treatment options for the disease. Physician researchers seek to understand how the immune system responds to COVID-19 and create a healthier Nevada. During the early stages of the pandemic, convalescent plasma was considered the only viable treatment option available for patients with COVID-19. Convalescent plasma is the component of the blood from recovered patients that may contain COVID-19 antibodies that help fight the infection. The National Institutes of Health has since developed treatment guidelines for COVID-19 based on clinical trial data and many studies are still underway worldwide assessing various additional treatment options. Convalescent plasma was in high demand but difficult to locate for COVID-19 patients in the northern Nevada area. A 24-year-old nursing assistant, Austin Meegan, was hospitalized and spent weeks staving off kidney and lung failure before learning he was eligible for an experimental blood transfusion that showed promise in treating COVID-19. Doctors estimated Meegan had only about a 3% chance of tracking down a donor to match his rare blood type. A COVID-19 survivor, Thomas Gibson, a Texas resident with the same blood type, traveled to Reno to donate his viral antibodies and a convalescent plasma donation credited with helping to save Meegan’s life. Physician clinical researchers and scientists at Renown and the University of Nevada, Reno School of Medicine (UNR Med) knew they needed to create better options for patients and physicians. Clinical researchers developed a study to help other patients like Austin, and pleaded for donations from recovered COVID-19 patients to donate their convalescent plasma. The researcher teams looked to understand how the body’s immune system responds to the virus over time, to aid them in developing new treatments for COVID-19. “The world’s capacity to get through the COVID crisis will depend on four things — science, technology, innovation and partnerships, says Tony Slonim, MD, DrPH, President and CEO of Renown Health.  “Taking lab bench discoveries to the bedside of patients in an efficacious and timely manner is not easy, but with UNR Med and our partners, we are making great strides in advancing clinical research which has the power to save lives and to create a healthier Nevada.” “It’s tremendously promising to partner on clinical research that will not only help us better understand the disease, but help inform treatment for those combatting COVID-19 that has had such a devastating impact on Nevadans, our nation and the world,” says UNR Med Dean Thomas L. Project coordinators at the Renown Research Office were overwhelmed by the community’s support and plasma blood donations. The research team is now asking area residents to participate in a study to analyze the efficacy of two COVID-19 tests. Participants will undergo two blood tests: one being a finger stick to provide results for a rapid test, and the other is a traditional venipuncture draw confirming the presence or absence of COVID-19 antibodies. This study is a collaboration with InBios International, Inc., a leading biotechnology company based in Seattle. Researchers are seeking: Individuals who have confirmed positive for COVID-19 and who have recently recovered from the virus. Study participants must be within 7-28 days from the onset of their symptoms. Individuals who have recently tested negative for COVID-19 and have never tested positive. Because there may be limited supply, the vaccine continues to be distributed in a tiered approach. An important pandemic milestone “The health and safety of our staff is our top priority, and we remain steadfast in our determination to fight the good fight for our community. Administration of the final doses of the vaccine is an important pandemic milestone as it brings an overwhelming sense of hope and relief to all of us at Renown,” said Tony Slonim, MD, DrPH, President & CEO of Renown Health. “The healthcare heroes vaccinated today have been batting this pandemic on the front lines and working around the clock for the past 11 months to keep our community safe. Administered 21 days after the initial dose, this dose marks the final dose of the vaccination series. Many Renown employees have also received the Moderna vaccine, of which second doses will be administered after 28 days.  The intricacies of vaccine administration Renown’s COVID-19 vaccination process includes pre-scheduling physicians and employees through the Epic MyChart electronic medical record used by Renown. Employees are masked, and in their personal vehicles, as they drive through the vaccination site at Hometown Health. Each physician and healthcare worker provides their verification, identification, documentation and consent. They then extend their upper arm to receive the vaccination. Following the receipt of the vaccination, they have a 15-minute observation period to assure they do not experience any immediate side effects.  Similar to receiving the first dose, these Tier 1A healthcare employees were encouraged to schedule their vaccination after their completed work schedule to have time to rest. As with all vaccinations, there is a potential for injection site reactions (redness, swelling and pain) as well as fever, fatigue, headache, chills, vomiting, diarrhea, muscle pain and/or joint pain. Fever is not an infrequent side effect, especially on the second dose, so these vaccinated employees will self-monitor their temperatures, as they cannot work at Renown with a fever. Renown invited media to help commemorate this triumphant vaccination event. Present during today’s event were- Dr. Bret Frey, representing the Medical Staff and Emergency Medicine physicians at Renown; Dr. Jacob Keeperman, Medical Director, Renown Regional Transfer and Operations Center representing physicians in Renown’s Intensive Care Unit; and Holly Irwin, a Clinical Respiratory Therapist. In order to quickly vaccinate additional residents of Washoe County age 70 and older, Renown Health has partnered with the Washoe County Health District to help distribute the initially available, limited supplies of COVID-19 vaccines. Due to the unprecedented demand of local residents, Renown now has a full “waiting list” for those interested in the COVID-19 vaccine, and will be working through the list to offer appointments as they become available.  Renown continues to vaccinate physicians and its essential workforce for COVID-19, and to provide nasal swab testing for those with a lab order from their provider, for COVID-19 symptoms. In addition, Renown has stretched to support the Washoe County Health District to meet the needs of those aged 70 and older who are now eligible to receive a COVID-19 vaccine.  Over this past weekend, more than 3,000 of Washoe County’s oldest residents were able to obtain an appointment, and receive their first dose of the COVID-19 vaccine at Renown’s Drive Thru COVID-19 vaccination clinic. Renown posted 3,000 appointments for those on the MyChart electronic waiting list and 100% of appointments were booked within an hour. “We want to help the Health District, and we are working to vaccinate as many people as possible, given our limited staffing, volunteers, space and vaccine,” says Melodie Osborn, RN, Chief Nursing & Quality Officer for Health Services. “We are glad to step in and help; and our goal is to always provide an exceptional vaccine experience. We ask the public to be patient with us, and to help support us in this important work.” Scheduling limited Both the Pfizer and Moderna COVID-19 vaccines require two doses, the second administered three to four weeks after the first. Renown is working to be sure those who receive their first dose also obtain their second dose in a timely manner. This includes 7,000 physicians and health care workers, and will now include those older adults vaccinated with their first dose. The complexity of the vaccine dosage requirements continues to draw down on the number of vaccines and appointments available.  Renown MyChart and phone systems strained under the demand Last Thursday, when Renown announced that those who are 75 years or older could join the waiting list by downloading Renown Epic MyChart, so many people logged in that the system was strained to accommodate the additional demand. Renown has now doubled the capacity of the Epic MyChart system to be sure all goes smoothly.  Renown Health has been able to distribute and administer the COVID-19 vaccine to a record number of people in the shortest possible time. Much of this success is due to the use of Epic MyChart, a convenient patient electronic medical record which allows people to schedule their appointments online, complete the consent form and have full documentation for the future. In addition, the information automatically posts to Web IZ Nevada’s Statewide Immunization Information System. Also last Thursday, older adults without cell phones, computers or someone to assist them, called Renown’s customer service phone line for assistance. So many people called the phone line that it crashed. Service has been restored, but is limited, due to the hundreds of calls coming in daily that are answered by a limited number of trained staff members. Stage 2 Air Quality Index and alarming research studies from DRI, Renown Health and the Washoe County Health District leads to public health advisory- seek care if needed For people who suffer from asthma, wildfire smoke is more hazardous than other types of air pollution, according to a study from the Desert Research Institute (DRI), the Renown Institute for Health Innovation (Renown IHI) and the Washoe County Health District (WCHD). The study published in the journal Environmental Health examined associations between airborne particulate matter (PM) from sources such as wildfire, transportation and industry, and medical visits for asthma at Renown Health’s emergency departments and urgent care centers in Reno, Nev. during the six-year period from 2013-2018. In places like Reno, where wildfire events occur regularly during parts of the year and are expected to become more frequent in the future, an accurate understanding of the impacts of wildfire smoke on population health is critical. We have an exceptional process in place through our urgent care sites, Renown Medical Group offices and hospital emergency departments to screen, diagnose and treat members of our community who come to us for care related to asthma, chronic obstructive pulmonary disease (COPD), chronic bronchitis, emphysema, and other upper respiratory diseases that may be exacerbated by wildfire smoke. At the same time, we continue to develop and refine, in real-time, the data that supports our predictive analytic models. We are using every tool and resource-including scientific data and clinical reporting, to ensure that we are meeting both the health and healthcare needs of the people we serve. In the study, researchers found that air quality in the Reno area was affected by wildfire smoke on a total of 188 days during the study period. A total of 18,836 asthma-related emergency room and urgent care visits occurred over the same five-year period of time, indicating that the influences of wildfire smoke and other types of air pollution on this medical condition are important to understand. The Washoe County Health District – Air Quality Management Division (AQMD) has issued a Stage 2 Air Pollution Warning due to smoke from area wildfires. Air quality in the Reno-Sparks area is now “Very Unhealthy” with an Air Quality Index of 247. As the need for Medical Assistants grows, graduating Hug High Health Science Academy senior, Ana Hernandez Rodriguez, starts NHSE sponsored Renown Medical Assistant apprenticeship at TMCC According to the U.S. Bureau of Labor Statistics, employment of Medical Assistants is projected to grow 19 percent from 2019 to 2029, much faster than the average for all occupations. The growth of the aging baby-boom population continues to increase demand for preventive medical services here in Nevada and across the country. An increasing number of group medical practices, clinics, and other healthcare facilities also need medical assistants to perform administrative and clinical duties. Medical assistants work mostly in primary care, a steadily growing sector of healthcare. With an eye towards this future, Renown Health proudly accepted its first student, Ana Hernandez Rodriguez, a graduating senior from Procter R. Hug High School’s Health Science Academy CTE program in Washoe County, into its newly created Medical Assistant apprenticeship program. Ana will begin the program as a full-time employee with benefits and start her apprenticeship coursework at Truckee Meadows Community College (TMCC). Sponsored by the Nevada System for Higher Education (NSHE), Renown’s Medical Assistant Apprenticeship Program was approved by the Nevada State Apprenticeship Council, which was overseen by the Governor’s Office of Workforce Innovation (OWINN), in February. The Renown Medical Assistant Apprenticeship is a one-year competency-based program that includes up to 2,000 hours of On-the-Job training and 193 hours of Related Technical Instruction offered through TMCC. All classroom, clinic and work time is paid. Apprentices work under the supervision of a Medical Assistant, Registered Nurse, or medical provider to assist in responsibilities including escorting the patient from the waiting room to the examination room, taking vital signs, stocking supplies, placing and receiving phone calls including appropriate documentation in the Electronic Medical Record, preparing lab specimens for processing, completing the necessary referral forms, obtaining authorizations and keeping records for referrals. Renown Urgent Care team prepares you to stay alert and safe this season and is available for medical assistance, 7 days a week at 11 locations across northern Nevada. In June, staffers at the Centers for Disease Control and Prevention sent a Health Advisory requesting that clinicians and caregivers watch for the respiratory syncytial virus, usually known as RSV, an infection that puts thousands of toddlers and senior citizens in the hospital each year with pneumonia and deep lung inflammation. According to the CDC, RSV results in roughly 58,000 hospitalizations and 100 to 500 deaths each year among children under five as well as 177,000 hospitalizations and 14,000 deaths among adults 65 and above. RSV was cropping up in 13 southern and southeastern states, the agency warned, and clinicians should test for the virus if kids showed up sneezing, wheezing, or with poor appetites and inflamed throats. Usually a winter infection, RSV is usually gone by June. Instead, this year, it is spreading—and has since continued to spread across the country, and to northern Nevada. “We know that many of the good hygiene habits we developed to defend against COVID-19 disrupted the viral landscape over the past 16 months, suppressing infections from almost every pathogen. Now RSV’s out-of-season return tells us that we need to continue to be on the alert,” says Robert Thole, MD, a physician with Renown Urgent Care. “We are surprised to be seeing so many kids with RSV right now.” “We ask parents and caregivers to keep a watchful eye out for a respiratory syncytial virus (RSV) infection. Symptoms are usually mild and resemble those of the common cold. RSV infection sometimes leads to bronchiolitis, pneumonia, or both. Symptoms of these complications include; difficulty breathing; wheezing; coughing that is getting worse; lethargy, increased tiredness, decreased interest in surroundings, or loss of interest in food,” explains Dr. Thole, who is board certified in family medicine. Catching and spreading the virus People with RSV infection may spread the virus through their secretions (saliva or mucus) when they cough, sneeze, or talk. You can catch the virus by: Touching an object or surface contaminated with the virus and then touching your nose, eyes, or mouth without first washing your hands. The virus can survive for more than 6 hours on countertops and other hard surfaces, such as doorknobs, and for 30 minutes on hands, clothing, or tissue. Close contact. If an infected person coughs or sneezes near you, you could breathe in RSV that is in his or her saliva or mucus. The virus spreads easily in crowded settings, such as childcare facilities, preschools, and nursing homes. Children attending school often spread the virus to their parents and siblings. The incubation period—the time from exposure to RSV until you have symptoms—ranges from two to 8 days but usually is 4 to 6 days. People are most likely to spread the virus within the first several days after symptoms of RSV infection begin and remain contagious for up to 8 days. Babies and young children may spread the virus for at least 3 to 4 weeks. Most children and adults feel better after a week or two. Treatment For an otherwise healthy child who has symptoms of an upper respiratory infection, such as a cough or runny nose, home treatment usually is all that is needed. It is important to watch for signs of complications, such as dehydration. Watchful waiting may not be a good choice when your child with an upper respiratory infection has an increased risk for complications. Watch your child closely if there are symptoms of an upper respiratory infection. If symptoms get worse or new symptoms develop, see a doctor right away. There is no vaccine and few specific treatments aside from supportive care, such as administering pain relievers and fluids. In the most severe cases, some children or adults might need additional oxygen. Not just in kids, older adults at risk Dr. Thole explains, “RSC is not only seen in children. Adults older than 65 have an increased risk of complications following infection with RSV. Pneumonia is a particular risk, especially if other health problems exist, such as chronic obstructive pulmonary disease (COPD) or heart failure. It may take older adults longer to recover from RSV infection and its complications than people in other age groups.” Call 911 or other emergency services immediately if: Your child has severe trouble breathing. Your child's breathing has stopped. Start rescue breathing. Call your doctor now if your baby or child has moderate trouble breathing. Call a doctor if your child: Breathes slightly faster than normal and seems to be getting worse. Most healthy children breathe less than 40 times a minute. Has cold symptoms that become severe. Has shallow coughing, which continues throughout the day and night. Has a poor appetite or is being less active than usual. Has any trouble breathing. Hospitalization Doctors sometimes give a monoclonal antibody treatment to high-risk babies throughout the respiratory season to prevent them from catching the virus or developing a severe infection. Some hospitals have started to increase use of the prophylaxis in response to the recent uptick in RSV cases. “Those kids were being protected,” said Kristina Deeter, MD who was quoted in a July 1st Wall St. Journal article on this topic. Dr. Deeter is a pediatric intensivist and medical director at Renown Children’s Hospital Pediatric Intensive Care and Hospitalist Medicine. Dr. Deeter, who also oversees Pediatrix Medical Group’s critical-care team in dozens of pediatric intensive care units across the country, said hospitals are also considering taking additional precautions in the neonatal intensive-care unit similar to some wintertime protocols. At Renown Children’s Hospital, there were no children hospitalized with RSV throughout the winter season, Dr. Deeter said, when there are normally hundreds of cases in our county- many of which do not need to be hospitalized.” A total of 31 cases have been reported in Washoe County, Nev., since April, and 26 of those occurred within the past three weeks. Preventing infection To prevent infection, or at least to mitigate them, wear masks, stay home when sick, and washing hands could make a difference in how the next viral season unfolds.
