Renown Health announces the appointment of Ernesto P. Molmenti, M.D., PH.D. M.B.A. as the hospital’s inaugural kidney transplant surgeon. Dr. Molmenti comes to Renown from New York City. Dr. Molmenti is a highly skilled and experienced transplant surgeon specializing in kidney transplantation. He has 25 years of experience in performing successful kidney transplant surgeries and is dedicated to improving patient outcomes and quality of life. The need for additional organ transplantation expertise across the U.S. and Nevada is critical. Across the U.S.,17 people die each day because an organ does not become available to them while they are waiting and every 8 minutes someone is added to the waiting list. Kidneys are the most sought-after organ for transplantation. UNOS (United Network for Organ Sharing) reports that there are 89,101 people on the national kidney transplant waitlist. Because of the lack of transplant surgeons and organ availability, only 25,499 kidney transplants were performed in the U.S. last year. Before joining Renown, Dr. Narisorn Atsava-Svate was a transplant nephrologist with INTEGRIS Nazih Zuhdi Transplant Institute, a leading solid organ transplant program based in Oklahoma City. While there, he helped build one of the largest and most experienced transplant programs in the U.S. and completed over 2070 kidney transplantations as part of a comprehensive team. Dr. Nate, as he likes to be called, received his medical degree from the oldest and largest medical school in Thailand, Siriraj Medical School/Mahidol University in Bangkok. Study From UNR Med and Renown in JAMA Network Open Highlights Gaps in Genetic Screening for Hereditary Breast and Ovarian Cancer
Data demonstrates that a proactive approach to family history assessment and genetic screening will increase early detection of inherited risk. A new original investigation article published in JAMA Network Open, “Screening Familial Risk for Hereditary Breast and Ovarian Cancer,” by authors from University of Nevada, Reno School of Medicine, Renown Health and Helix for the Healthy Nevada Project® emphasizes the critical need to increase genetic screening in identifying individuals at increased risk of breast and ovarian cancer due to family history. This landmark study calls for enhanced awareness, the adoption of genetic screening practices, and, importantly, the direct screening of family history by individuals using a simple tool such as the FHS-7 questionnaire. Breast and ovarian cancers remain leading causes of cancer-related deaths, with genetic factors playing a key role in up to 10% of cases. The study authors advocate for wider adoption of screening protocols, simple family risk questionnaires and education about the benefits of genetic testing, not only for patients but also their family members who may share similar genetic risks. Breast cancer is a leading cause of cancer death among women in the United States. According to the American Cancer Society, about 1 in 8 women will develop breast cancer and about 1 in 36 women will die from breast cancer. Since 1990, breast cancer death rates have declined progressively due to advancements in screening, treatment and detection. Here in Washoe County, Nev., the age-adjusted mortality rate for 2020 (last update) is 21 deaths/100,000 females due to breast cancer, which is higher and worse than the national U.S. value of 19.6. Nevada ranks near the bottom in breast and cervical cancer deaths in women according to a recent Commonwealth Fund report. Across the U.S., approximately 1.1 percent of women will be diagnosed with ovarian cancer at some point during their lifetime. The research underscores that individuals with a family history of breast and ovarian cancer have a higher likelihood of carrying genetic mutations in genes such as BRCA1 and BRCA2, which significantly elevate their cancer risk. However, many at-risk individuals remain unaware of their genetic predispositions due to limited screening practices. Evidence suggests that “up to 80% of patients with a pathogenic or likely pathogenic (P/LP) variant in the BRCA1 and BRCA2 genes are unaware of their condition and subsequent risk for hereditary breast and ovarian cancer. Rare or less penetrant P/LP variants in genes such as PALB2, CHEK2 and ATM are even less likely to be detected. This is concerning because the lifetime risk for breast cancer exceeds 50% for women with P/LP variants in BRCA1/2 and 20% for women in P/LP variants in PALB2, CHEK2 and ATM (compared with a baseline risk of 12%).” The study also found that men with family history are just as likely to have pathogenic variants in BRCA1 and BRCA2 as women. This is important because not only are men at increased risk of breast, prostate and other cancers, these risks are passed on to daughters, nieces and other members of the family through inheritance. Men are under-represented in the Healthy Nevada Project® and other similar population health studies. Daniel Kiser, MS, lead author of the study stated, "Our work shows that family history collected during routine medical care can be evaluated using simple criteria and used to identify patients with elevated risk for inherited cancers. Because there are a number of steps that can be taken to reduce risk, both for the patient and for their family members, following up with these patients has the potential to substantially reduce mortality and the incidence of cancer." “Prevention or early detection of disease is such an important part of healthcare,” says Bill Plauth, MD, MMM, CPE, Chief Medical Officer for Renown Health and Associate Dean for Clinical Affairs at the University of Nevada, Reno School of Medicine. “The Healthy Nevada Project® is leading the way in helping Renown Health physicians and clinicians assess family history risk of breast and ovarian cancer syndrome. This in turn, enables us to provide our at-risk patients the best available tools for prevention or to catch cancer at an earlier stage, which affords less aggressive treatment options with significantly improved survival.” The study found that routine genetic screening for individuals with a strong family history of these cancers could lead to more personalized and effective healthcare strategies, including preventative surgeries, increased surveillance, or early interventions. It also highlights the gap in awareness and access to genetic testing, particularly in underserved populations. “One of the lessons from our work is the importance of assessing family history early and often in the healthcare journey of patients,” says Joseph Grzymski, PhD, Chief Genomics Officer of Renown Health, the Principal Investigator of the Healthy Nevada Project, and a faculty member in the Department of Internal Medicine at the University of Nevada, Reno School of Medicine. “We recommend that everyone get screened for CDC Tier 1 conditions but particularly, when family history indicates risks, genetic screening is a powerful tool for risk stratification.” Key Recommendations from the Study Authors Include: Increased Engagement of Patients with Physicians: People with a family history should discuss their history with their primary care physician or health care provider and request a genetic screening. This proactive approach to prevention will save lives. Increased Access to Genetic Testing: The need for more accessible and affordable testing options, like the no-cost screening provided by the Healthy Nevada Project®, particularly for those with a family history of breast or ovarian cancer. Increased Pre-Screening and Genetic Counseling: Providing patients with the necessary counseling to understand the implications of genetic mutations and guide their healthcare decisions. Pre-screening patients with either a personal or family history of cancer with one of several simple family history tools, such as the FHS-7 Questionnaire to determine whether they should be referred for genetic services would also help increase early identification and reduce costs. As of January 2024, the Healthy Nevada Project® had notified 330 individuals with P/LP variants in BRCA1 or BRCA2 genes of their genetic risk, of a study population of more than 53,000 patients. Renown and Great Basin College Launch 18-month Respiratory Therapist Training Program
Training respiratory therapists as part of a healthcare team to help diagnose, treat and manage conditions to help restore or maintain lung and breathing function. Applications due Oct. 31, 2024 Great Basin College and Renown Health are partnering to bring the first Respiratory Therapy training program to educate new healthcare professionals to meet the growing needs of the community. With the generosity of grant funders, the Leona M. and Harry B. Helmsley Charitable Trust and the Health Resources and Services Administration, Great Basin College will launch the first and only available program of its kind in support of several rural northern Nevada communities. Today, the University of Nevada, Reno School of Medicine (UNR Med) and Renown Health announced the opening of a new satellite location of the Savitt Medical Library at Renown Regional Medical Center. This new space will serve as a valuable resource for UNR Med students and Renown Health employees and physicians. The Savitt Medical Library's new satellite library will be accessible 24/7 to all Renown employees, physicians and UNR Med students who possess valid badges. Where There is Smoke; There May Be Headaches, Nausea and Coughing
Renown Urgent Care and Emergency Room teams encourage you to stay safe. Urgent Care is available seven days a week at 10 locations across northern Nevada and Emergency Rooms 24/7. Millions of people across the West live in areas where air pollution can cause serious health problems. Local air quality can affect our daily lives. Like the weather, it can change from day to day. Northern Nevada Public Health’s (NNPH) Air Quality Management Division (AQMD) has issued an Air Quality Advisory due to the Davis Fire burning in southern Washoe County. Air monitors for the most region hit the “Moderate” range Saturday night, which is 151-200 on the Air Quality Index (AQI). That means some members of the public may experience health effects. All residents in impacted areas are encouraged to regularly monitor the air quality nearest them at www.airnow.gov and limit outdoor exposure when they see or smell smoke. Exposure to elevated PM2.5 (fine particulate matter in smoke) concentrations can result in eye and throat irritation, headaches, nausea, shortness of breath, congestion, coughing, impaired lung function and chest pain, especially among sensitive individuals such as the elderly, children, people with asthma, people with heart or lung conditions, pregnant women and anyone who is exercising or working hard outdoors. Aleise Whitlock, APRN, Section Chief for Renown Urgent Care advises that there are people more likely to be affected when fine particle pollution reaches an unhealthy level, including: People with asthma or breathing conditions like Chronic Obstructive Pulmonary Disease. People who have heart disease or high blood pressure. Children and older adults. People doing extended or heavy physical activity like playing sports or working outdoors. “Everyone should take precautions when the air quality is unhealthy. Air pollution can aggravate heart and cardiovascular disease as well as lung diseases like asthma and COPD. When the air quality is unhealthy, people with these conditions may experience symptoms like chest pain, shortness of breath, wheezing, coughing, or fatigue. If you are experiencing any of these symptoms, use your inhalers as directed and contact your health care provider,” says Ms. Whitlock. “If you do not have a health care provider, we are happy to see you. Renown Urgent Care offices are on almost every corner, with providers ready to see you.” She advises to take it easy and listen to your body, and: Limit, change, or postpone your physical activity level. Drink plenty of water. Stay away from local sources of air pollution like busy roads and wood fires. Use a damp cloth or mask to reduce smoke inhalation if you must go outside. If you have asthma or other breathing conditions like COPD keep your relief/rescue inhaler with you. People with asthma should review and follow guidance in their written asthma action plan. Make an appointment to see your health provider to be sure you have an asthma action plan. When to Visit the Emergency Department Bret Frey, MD, President, Western Emergency Physicians and Renown Health Emergency Departments, says, “Seek emergency care if you or someone you know is experiencing any of the following symptoms due to smoke exposure: Severe shortness of breath or difficulty breathing-If breathing becomes labored or painful, you cannot catch your breath even after resting, this may be a sign of serious respiratory distress. Chest pain or tightness- Any discomfort in the chest, particularly if it is associated with breathing, should be evaluated immediately. Persistent coughing or wheezing- If a cough doesn’t improve or worsens, or if you develop wheezing, you should seek emergency medical attention. Confusion, dizziness or fainting- These can be signs of oxygen deprivation, and immediate care is critical. Bluish lips or face- A clear indicator that oxygen levels are dangerously low and emergency medical treatment is needed. Burns or smoke inhalation from a fire- Individuals who have been exposed to fire, or smoke in a confined space, should be evaluated for potential lung damage and other complications. “Even just one hour of smoke exposure can affect our lungs and hearts, and these consequences can reach people far beyond the direct area endangered by the fire. Children, older adults, pregnant women, people with preexisting cardiopulmonary disease, communities of color, economically disadvantaged communities, and people with preexisting chronic health conditions like asthma, diabetes, and obesity are all especially vulnerable to the air pollution that wildfires cause," says Dr. Frey.
