    What is Care Coordination for Senior Care Plus Members?

    Cost-saving isn’t the only reason to enroll in a Medicare Advantage Plan. One of the main reasons Medicare beneficiaries in Nevada join a Senior Care Plus Medicare Advantage Plan is for the care coordination services. The Senior Care Plus Care Coordination team helps members navigate what can be a complex healthcare system. Care coordination is a popular and extremely important service for members because keeping members healthy is the number one goal. One way they help reach this goal is to encourage members to participate in a no-cost, comprehensive health assessment. At this Quick Start Health Assessment, members meet with a geriatric specialist – a provider who specializes in the care of seniors – to discuss the 4 Ms: Mentation – Thinking, memory and mental health Medications – Understanding your medication Mobility – Staying physically active What Matters to You – Let your provider know what is important to you – examples could be family, health and independence The results of this detailed visit are then shared with the member’s primary care provider, so a customized care plan can be developed. This is a free service for Senior Care Plus members, along with an annual wellness visit and an annual physical exam. Care Team Approach – Laying the Foundation to Improve Health Health assessments and annual visits are offered so Renown Health providers can build relationships to improve care. This approach, also known as the Building Relationships to Improve Care or BRIC Model, is the care model used across Renown Health. “What’s special about this care model is that it really puts our patients at the center of their care,” says Savannah Gonsalves, a registered nurse with Senior Care Plus. “Members have their providers and nurses, Senior Care Plus personal assistants, case managers, and within the BRIC Model, they’re all talking to one another and putting the focus on the patient to meet needs.” Personal Assistants – A Unique Connection to Each Member A team of personal assistants is available to help members coordinate care by: Scheduling a member’s appointments Answering a member’s benefits questions Helping navigate care – these are experts in both health insurance and healthcare Answering questions about medications Working with providers to coordinate a member’s care The Senior Care Plus personal assistants are one of the most popular services that the Medicare Advantage plan offers. Each personal assistant has a direct phone line so members can call them to ask questions. “After my hip surgery my personal assistant, Megan checked in on me every day,” recalls Janelle, a Senior Care Plus member. “She made sure that I was doing alright and that I didn’t need anything.     New Renown Health Scholarship Supports UNR Med Students - The Next Generation of Physicians and Health Care Professionals

    Clinical affiliation to create more access to care for Nevadans. View the original press release by the University of Nevada, Reno School of Medicine. Nevada continues to face a physician shortage ranking No. 45 in the United States for active physicians per 100,000 population and No. 48 for primary care physicians per 100,000 population according to the Association of American Medical Colleges (AAMC) 2019 Nevada Physician Workforce Profile. A new clinical affiliation between the University of Nevada, Reno School of Medicine (UNR Med) and Renown Health addresses the provider gap and increases access to care by developing new and innovative ways to train and retain the next generation of Nevada's health care professionals. A cornerstone of the affiliation is the Renown Health Scholarship, which was established by Renown to support UNR Med medical students and physician assistant students in their pursuit of a career in health care. The goal of the scholarship is to award $350,000 annually based on need and merit. In 2021-2022, the scholarship will be awarded to 20 first-year and third-year medical students, each receiving $15,000 for a total of $300,000; and five second-year physician assistant studies program students, each receiving $10,000 for a total of $50,000. "Students are at the core of what we do as a School of Medicine, and this incredibly generous scholarship is the best way possible for Renown Health to celebrate our partnership and support our students," says Melissa Piasecki, acting dean of UNR Med, a role that includes Chief Academic Officer at Renown Health. "We thank Renown Health for this gift that will benefit so many of UNR Med students on their journeys to becoming great doctors and physician assistants. "We are extremely proud to establish the Renown Health Scholarship with UNR Med providing annual support to students pursuing careers in health care," says Anthony D. The inaugural group of scholarship recipients epitomize Renown Health's and UNR Med's combined vision of A Healthy Nevada and mission of improving the health and well-being of the communities they serve. The students chosen for this scholarship further represent UNR Med's commitments to excellence and to a culture of diversity and inclusion in support of their goals of becoming physicians and physician assistants. "This scholarship will contribute greatly to my becoming a physician that can later serve those from lower socioeconomic statuses. I am humbled and honored by the fact that Renown believes that my hard work and determination are worth rewarding, I know it will alleviate some of the mental stress that finances play in my life, and I hope to make these funds count through hard work and dedication to my studies," says Eunice Aiyuk, first-year medical student at UNR Med, who was born in Yaounde, Cameroon, in West Africa, and has lived in Reno since she was three years old. Aiyuk graduated from McQueen High School and from the University of Nevada, Reno with a bachelor of science degree in neuroscience in 2018. Sergio Trejo, Jr., MPH, is a third-year medical student who grew up in Las Vegas and served as a Spanish interpreter at a community health center prior to earning his Master of Public Health degree from the University of Nevada, Reno in 2019, which instilled in him his desire to pursue a career as a primary care physician in pediatrics. "I'm interested in a career in pediatrics since I can combine my knowledge of medicine and public health to not only influence the health of children, but also the entire family, as they are ones that ultimately dictate their child's health outcomes," Trejo says. "This scholarship means the ability to fund my dream of helping alleviate health disparities in underserved communities, especially those who endure language barriers. It will help reduce the stress of funding resources that will help me be successful in my clerkships and beyond." As a second-year Physician Assistant Studies Program student at UNR Med, Benjamin "Benjie" Portillo-Dominguez's path to becoming a physician assistant started at a young age when he attended his parents' doctors' appointments so that he could interpret. He recalls his family's excitement when a health care provider could speak Spanish. "It made a world of difference because it maximized communication and helped them receive the care they needed. This helped develop in me a strong desire to pursue a career in health care, says Portillo-Dominguez, who graduated from the University with a bachelor of science degree in community health sciences in 2018. "In less than a year I will become a Physician Assistant. This has been a long-time goal of mine, but as a first-generation college graduate, this has been the dream of my parents since I was born." "Receiving this scholarship benefits me in my training because it allows me more time to focus on my studies and opens up opportunities to learn from experienced providers. Becoming the best provider I can be, allows me to strengthen this community that I was born and raised in. Having this unique perspective and being in this program allows me to be a solution to the lack of primary care providers in this area, especially with those who speak Spanish." The UNR Med student recipients of the 2021 Renown Health Scholarship were based on merit and need. They include: first-year medical students Eunice Aiyuk, Dakota Johnson, Aisha Masud, Isabella Rodriguez and Raquel Wescott; third-year medical students Davison Beenfeldt, Sara Bertram, Kaileigh Bingham, Curtis Duncan, Brooke Gantman, Alex Harper, Erin Kelley, Regina Kurapova, Ngantu Le, Otto Maurer, Keanu McMurray, Eric Nguyen, Hao To, Sean Torres and Sergio Trejo; and second-year physician assistant studies program students Ali-Marie Lostra, Margaret (Meg) Patterson, Benjamin (Benjie) Portillo-Dominguez, Preston Reugebrink and Ryan Swanson. The UNR Med/Renown Health affiliation is between a 159-year old health care provider, the state's original land grant university and the state's oldest medical school. Think of discovering a diagnosis like solving a mystery: the condition is the suspect, the nurses are the frontline police force and the doctors are the lieutenants or captains finalizing the results of the case. You may notice that one crucial role is missing on this list – the detectives. In the diverse network of healthcare, the detectives are a significant part of each patient’s mystery-solving care team and represent many roles across our health system. When it comes to figuring out the elaborate details of a growth, disease, organ abnormality or cause of death, one team of detectives, quite literally, goes as deep as possible. Those detectives are the team members within Renown Pathology. For each specialized field within medicine or surgery, the Pathology department is here to play a crucial role in accurate diagnoses. With each slide examined and each test meticulously conducted in their bright laboratories, these dedicated professionals shape a path towards wellness and recovery.  Meet Your Anatomy Experts  Whether you have a chronic disease that needs consistent testing, a high-risk birth that requires placenta testing, a suspected cancerous tumor that needs a biopsy or a gall stone that must be removed (or anything in between), Renown’s Pathology team steps in to provide biological answers to your body’s questions. This department offers the most comprehensive in-house diagnostic testing in the region, from routine histology to full pathology.  As the busiest pathology department in northern Nevada, this team boasts the fastest turnaround times from respected experts, including:  Pathologists Pathology Assistants Histotechnicians Histotechnologists Clinical Lab Assistants Let’s break down the complex nature of these team members' jobs by walking through their everyday responsibilities at work!  Pathologists  Pathologists are medical doctors who specialize in the study and diagnosis of disease. With every slide they scrutinize and every sample they analyze, pathologists unravel the mysteries of disease with precision and compassion. Their responsibilities include interpreting laboratory tests, analyzing tissue and fluid samples (obtained from a variety of different sources, including biopsies and surgeries), staging cancer diagnoses and providing diagnostic insights that guide treatment decisions.  “Our job is to help the patients and their doctors figure out what’s wrong,” said Dr. Christie Elliott, Pathologist and Medical Director of the Clinical Laboratory at Renown Regional Medical Center. “As the bulk of our cases deal with cancer, almost every day we start with a tumor board alongside fellow surgeons, oncologists, radiologists and geneticists. From there, we order extra studies, run through our cases to make diagnoses, review slides and ensure all information goes into the charts, which is especially important as 70% of data in medical charts is from the lab. A patient’s history is everything.”  Pathology Assistants With the steadiest of hands, pathology assistants, also known as PAs (not to be confused with physician assistants), guide the diagnostic journey from patient specimen to diagnosis. They can typically be found processing surgical and biopsy specimens (includes accessioning, gross examination, description, and sampling for microscopic analysis), preparing tissue samples for microscopic evaluation, helping the pathologist determine a cause of death for autopsies by conducting organ dissections and maintaining detailed records of all diagnostic findings.  “As a PA, I still impact patient care without being directly patient-facing,” said Andrew Whitner, Pathology Assistant. “I handle 300-350 small tissue blocks a day. During dissections, I identify landmarks, document what I see and turn those landmarks into slides, looking for things that don’t look normal.”  “Our job is 90% all about gross specimens, and we also do eviscerations for autopsies,” added Leslieann Haffner, Pathology Assistant. “We are trained on what normal looks like; our goal is to find the abnormal.”  Histotechnicians Histotechnicians work behind the scenes to help transform ordinary tissue into extraordinary windows of insight, revealing the inner workings of the human body. As vital members of the Pathology team, histotechnicians embed tissue specimens in paraffin wax blocks (a process that preserves the tissue's structure for examination), cut thin sections of tissue from the paraffin blocks using a microtome, mount tissue onto glass slides and stain the tissue slides using histological stains to highlight structures or cells.  “With all the patient specimens we work with, we get to see a lot of organs and learn what is causing the abnormalities,” said Reiny Hitchcock, Histotechnician. “I enjoy the opportunities to expand my knowledge, especially while working alongside the doctors.”  “Our job can change by the week,” added Jessica Fahrion, Histotechnician. “One week I’ll be in the grossing room, and the next week I might be training in cytology." Histotechnologists In a world where every slide holds the key to a patient's future, histotechnologists are the champions of progress. One career ladder step above histotechnicians, these team members often have a broader scope of responsibilities, including more complex laboratory procedures, developing and validating new techniques, managing laboratory operations, interpreting results and troubleshooting technical issues. You can count on histotechnologists for validating antibodies and handling orders from pathologists, oncologists, emergency physicians and more.  “My day always involves looking into cases, reading reports, getting orders together and working with pathologists to help them with their diagnoses; I also work a lot with immunohistochemistry, helping out with routine slides,” said Charles Koeritz, Histotechnologist. “I especially enjoy doing validations, which help maintain the integrity of lab testing and our diagnostic processes.” Clinical Lab Assistants Our pathology clinical lab assistants are the masters at “filling in the blanks,” assisting in whatever area needs it most, especially in cytology and the grossing room. They are essential aspects of the Pathology team, collecting and storing specimens for further testing, assisting in managing test results, gathering data, managing supply inventory and more.  “As a Clinical Lab Assistant, I can be scheduled anywhere, from tissue cassetting to grossing,” said Ellie Somers, Clinical Lab Assistant. “Working in cytology is one of my favorite parts of my job. It’s very rewarding to work with the doctors to uncover what treatments will help each patient. We do cytology very well here.” The Bottom Line Even though the Pathology department doesn’t always experience a lot of patient face-to-face time, they interact with patients in a different way – by uncovering the story that is the inner workings of the human body, one slide and one sample at a time.  “It’s important to remember that the slide IS a patient,” said Dr. Elliott. “We are constantly learning from every case so we can continue to provide the best patient care possible.”  Take a Photo Tour of the Pathology Lab!

From helping during procedures to providing education and guidance, these medical professionals are key in ensuring each patient’s visit – and as a result, their rehabilitation journey – is successful. “My normal day in Physiatry is very smooth,” said Rosita Gans, Medical Assistant. “I make sure my schedule is at 100% to prioritize patient needs. It helps that everyone is so respectful and friendly.”  Our patient access representatives (PARs) and physician scheduling coordinators are the front faces of physiatry. They serve as a patient’s first line of communication when checking in, making an appointment, handling insurance verifications and several aspects of follow-up – all with a smile on their faces.  “My job as a PAR involves checking patients in and out, verifying their insurance and demographic information, scheduling and following up on appointments, referrals to Special Procedures and more,” said Andrea Ruiz, Patient Access Representative. “I am in contact with patients on a daily basis.”  “It’s all about making sure every patient is taken care of in a timely manner,” added Kasey Sinfellow, Physician Scheduling Coordinator.  At the core of keeping things running, you’ll find Antonio Quintana, Practice Site Leader. Thanks to his expert leadership skills and can-do attitude, his department has been able to reach new heights. A large part of this team’s success lies in their approach to the practice of physiatry.
