Renown Health announced today that it is now using ARxIUM’s Robotic IV Automation (RIVA) system in its pharmacy to streamline the production process of IV admixtures. RIVA is a fully automated compounding system designed to precisely and accurately prepare IV syringes and bags in hospital pharmacies. This state-of-the-art system is considered the gold standard when it comes to safety, effectiveness and precision in pharmacy technology. To operate the robot, pharmacists and pharmacy technicians load drugs into the machine, in addition to drug vials, syringes and IV bags. Everything that goes into RIVA receives a barcode as it’s loaded to help the robot identify each item. The operator then instructs the robot to create the drug and RIVA goes to work, allowing employees to go about other duties as the robot maximizes efficiency. RIVA validates each component during the admixture process including size, barcode and weight of each ingredient to ensure accuracy. For an added level of safety, RIVA uses high-intensity pulsed ultraviolet light for disinfection of all vial and bag parts prior to compounding the drugs. “The contamination of IV products usually comes from humans – from our skin, breath, hair or other particles in the air,” said Renown Regional Medical Center’s pharmacy manager, Derek Gillespie. “Plus, the repetitive process of a human mixing 100 items of the same thing and the possibility for distraction can lead to errors. By automating the complex admixture process using the RIVA system, we’re able to significantly reduce the risk of contamination and human error.”  RIVA has the continuous capability to learn how to produce new admixtures through detailed programming done by pharmacists and pharmacy technicians. The system stores the information so it can recreate the drug whenever it’s instructed to do so.  With the help of pharmacy staff, RIVA produces a batch of approximately 50 items at a time, and can make up to 500 admixtures a day. By using RIVA, the pharmacy’s goal is to have items available on hand to help meet patients’ needs quickly. Additionally, in the case of a drug shortage, RIVA would enable the pharmacy to make its own IV solutions or admix items unavailable for purchase.  Only 27 pharmacies in the country currently use the RIVA system. The drug is especially important for individuals who have experienced a heart attack or stroke, or are at risk of experiencing either. However, the backstory of the drug is rather fascinating.  Beginning in the 1920s, an unknown disease was killing cattle throughout the U.S. and Canada. Desperate to find a solution, one farmer drove 200 miles to the University of Wisconsin with a dead cow, bales of spoiled hay and a milk can of unclotted blood in tow. He crossed paths with a scientist, whose team set out to determine the hemorrhagic component of the spoiled hay, resulting in a series of medical discoveries that would revolutionize medicine.  After learning more about Warfarin and how Vitamin K could reverse its blood thinning properties, doctors first prescribed the drug for human use in 1954. It is perhaps most widely known for saving President Dwight D. Eisenhower after he suffered a heart attack in 1955. Today, Warfarin continues to saves millions of lives each year. “Before blood thinners like Warfarin, people would experience strokes, blood clots in their lungs, blood clots in their legs, blood clots in their hearts, along with countless other clotting disorders - these conditions likely cutting their lifespans short,” Dr. Rowan said during the podcast. “Warfarin revolutionized the treatment of patients who suffer from heart attacks and strokes, helping them to live normal and healthy lives. Renown employees volunteered to reach out to over 2,500 older adults participating in the non-profits’ free Sterling Silver Club. While people of all ages are coping with the challenges of social isolation, there is one group of individuals who may feel especially lonely during these uncertain times—our aging adults. To offer some friendly conversation and answer any questions older adults may have about Renown’s commitment to keeping them safe during the COVID-19 crisis, Renown Health employees are making phone calls to its Sterling Silver Club members. To date, employees have made nearly 1,000 phone calls. Seven Troughs Distilling Co., Damon Industries, The Depot and Forsaken River have worked together to supply Renown Health with 726 gallons of hand sanitizer to date. Thanks to the support of local businesses, Renown Health’s need for hand sanitizer is being met. Seven Troughs Distilling Co., with the help of several area distilleries including The Depot Craft Brewery Distillery and Forsaken River Spirits, is using their distilling equipment to create ethyl alcohol. Damon Industries mixes the ethyl alcohol with other key ingredients in accordance to the hand sanitizer recipe issued by the FDA and World Health Organization, bottles the contents, and distributes the bottles to Renown. This community-wide collaboration has resulted in 726 gallons of hand sanitizer for Renown thus far. Seven Troughs had previously manufactured hand sanitizer and surface sanitizer using their non beverage grade alcohol for years, which employees used internally to sanitize their hands and clean machines. At the start of the pandemic, the distillery continued producing the sanitizer for family and friends. The word spread quickly about Seven Troughs’ capabilities – and that’s when Renown reached out to the distillery to see if they could help.  “All of a sudden, instead of making a 50-gallon batch of hand sanitizer, we needed to make a 500- gallon batch,” said Seven Troughs president Tom Adams. “We quickly realized that we were very much out of our depth. We thought it would be a better plan if we could focus on what we do well, which is making ethanol, and if we could get a partner that could focus on the formulation and bottling. That ended up being Damon Industries.”  Before the pandemic, Damon Industries primarily manufactured juice products for smoothie shops and healthcare facilities – a market that had basically collapsed. The team at Damon Industries was quick to respond to Adams’ call for assistance, as it was a chance to serve the community and keep staff employed. “It didn’t take any convincing,” said Damon Industries’ vice president of sales Brandon Halvorson when asked about the company’s impetus for moving into the hand sanitizer industry. “We knew the community needed a product and we knew we had the partnerships that could get it done. It’s been our pleasure to work with the outstanding people of Renown and of northern Nevada to keep our community supplied during this difficult situation.”  Before boxing up and distributing the hand sanitizer to Renown facilities, employees at Damon Industries stick a Renown-branded Fight the Good Fight label to every bottle, a reminder that the fight against COVID-19 extends far beyond the walls of Renown’s care sites. “I am amazed by how quickly Seven Troughs and Damon Industries sprang into action to meet our need for hand sanitizer,” said Renown’s director of operations Ian Wayman. “In addition to their swift manufacturing efforts, they’re selling us the hand sanitizer at an extremely reasonable rate, charging just enough to pay for supplies and keep their staff employed. Today, in accordance with recommendations from the U.S. Surgeon General and the American College of Surgeons, the Renown Health Joint Medical Executive Committee decided that effective Tuesday, March 17, surgical cases scheduled at both Renown Regional Medical Center and Renown South Meadows Medical Center, considered to be ELECTIVE-OPTIONAL will be postponed to a later date.  Patients with scheduled ELECTIVE-OPTIONAL surgeries will be called by Renown Health representatives over the next week. Surgeon’s offices will also be reaching out to assist patients with rescheduling their surgeries for a future date.  Elective surgeries that are REQUIRED and not optional will be evaluated on a case-by-case basis daily with the surgeon, patient and a medical leadership review committee at Renown. Your surgeon’s office will be in contact with you with more information about these REQUIRED surgeries This temporary measure will allow Renown’s physicians and staff to enact the emergency preparedness plans they have been developing for weeks to create additional capacity for inpatients and to continue to deliver high quality care during the anticipated surge in COVID-19 cases in northern Nevada. The new guidelines do NOT apply to EMERGENCY surgeries performed in Renown hospitals. There are different types of surgery, this decision impacts Elective-Optional surgeries only. Elective – Optional surgery – Elective surgeries are not usually necessary for the individual to stay in good health. They are non-emergency and planned in advance. A wide range of surgeries can be elective. Most cosmetic surgeries are elective. Other surgeries such as organ donation, scoliosis surgery, tonsillectomies and other minor surgeries can also be considered elective. Required surgery – Surgery which needs to be done in order to retain quality of life. As opposed to urgent or emergency surgery, required surgeries do not need to be performed immediately. Examples of required surgery are kidney stone or tumor removal. Urgent or emergency surgery – When a patient’s condition is life threatening, surgery is considered emergent. Emergency surgeries must be performed immediately, even when the patient is unconscious and cannot give consent. Examples include trauma and appendicitis. Life-threatening conditions can occur during labor and delivery which may require emergency surgery. Other services making changes include: Pulmonary Services and Procedures Renown Health today will begin rescheduling patients scheduled for Pulmonary Function Testing, Outpatient bronchoscopy and patients at Thomas S. Dolan Pulmonary Rehabilitation. Patients who are affected by this change are being called by Renown to reschedule their appointments for Monday, April20, 2020 or later. Starbucks at Renown and Renown Dining Options As with many of the Starbucks around the U.S., the Starbucks at Renown Regional Medical Center will shift to a “to-go” model, with no seating available. The cafeterias at Renown Regional Medical Center and South Meadows Medical Center remain open, will temporarily eliminate self-service options and will add more ‘grab and go’ options. Other Renown Services Remain Open Renown Medical Group offices remain open. Planned imaging procedures such as ultrasound, computed tomography (CT), magnetic resonance imaging (MRIs), mammograms and non-invasive cardiology testing will continue at Renown hospitals and office locations and will NOT be postponed at this time. Patients confirmed for imaging appointments will be contacted prior to their scheduled visits to identify those who may be at higher risk of COVID-19 infection. Alexander, M.D., an experienced leader and clinician in OB/GYN, is regularly in the room at the beginning of something new, so it’s fitting that he is one of the first jointly appointed senior clinical leaders of Nevada’s first integrated health system. As the chair of the department of Obstetrics and Gynecology at the University of Nevada, Reno School of Medicine (UNR Med) and Renown Health, Dr. Alexander will lead the medical and surgical services, academic advancement, research, and teaching programs for the healthcare system. He was recruited to the faculty at UTSW after finishing his fellowship and held several leadership positions there, including chief of the obstetrics service. Dr. Alexander established himself as a clinical investigator at UTSW and served as co-investigator on several studies conducted by the Maternal Fetal Medicine Units Network of the Eunice Kennedy Shriver National Institute of Child Health and Human Development. He has published widely on obstetrics and maternal fetal medicine topics, including cesarean delivery, prematurity, obstetric complications, labor management, and obstetric anesthesia. He has contributed to the leading textbook on obstetrics, “Williams Obstetrics.” Following his tenure at UTSW, Dr. Alexander held the positions of division director of maternal fetal medicine at the University of Arkansas for Medical Sciences as well as director of maternal fetal medicine and obstetrics at the Billings Clinic.
