Northern Nevada Sierra Medical Center (Reno, NV) closed their inpatient pediatric unit for infants and children on June 1. This follows the 2022 decision by St Mary’s Medical Center (also in Reno) to close their 24-bed Neonatal Intensive Care Unit (NICU) and 18-bed pediatric floor in addition to eliminating maternity and birthing services. Why Are Hospitals Closing Pediatric Units? “Across the nation, pediatric units are being shuttered in favor of more lucrative units for adult care,” writes Brenna Miller, a Health Communications Specialist at the Lown Institute in her blog, Pediatrics and Profits: Why children’s hospital units are closing (Dec. 2022). “Why is pediatrics a drain on hospital finances? One reason is reimbursement. Medicaid, the state-run health insurance program for low-income patients, covers more than one third of children in the US, and Medicaid reimbursement rates are typically lower than those from other insurers.” The New York Times also reported a nationwide wave of hospitals shutting down their pediatric units in favor of more profitable adult units. “Adult beds are more lucrative than children’s beds. So as institutions look to boost profit margins, pediatrics is often among the first services to be cut,” said Emily Baumgaertner, Oct. 11, 2022. “The decline of local access to children’s inpatient care began over a decade ago and accelerated during the pandemic. Extracorporeal Membrane Oxygenation technology (ECMO) for patients with severe heart or respiratory failure is now available locally thanks to a grant from The Leona M. and Harry B. Helmsley Charitable Trust  Renown Health, the Level II Trauma Center for the region and the leader in offering the most innovative treatments for critically ill adults and children, is now home to northern Nevada’s first and only comprehensive extracorporeal membrane oxygenation (ECMO) program.  The ECMO machines arrived at the Cardiac ICU at Renown Regional Medical Center this week, and the multidisciplinary team of highly specialized physicians, Critical Care intensivists, Cardiovascular surgeons, perfusionists, registered nurses and therapists are ready to provide minute-by-minute support for patients receiving ECMO treatment. The multidisciplinary ECMO team includes pharmacists and respiratory therapists who are ready to receive their first patient. “At Renown Health, we are constantly investing in our clinicians, essential services, and medical innovations to help keep care local. The ECMO machine is a portable life-support system used for severe cases of heart and lungfailure. The ECMO procedure provides minimally invasive heart-lung bypass support outside of the body. The machine then removes carbon dioxide from the patient’s blood and adds oxygen. Basically, ECMO serves as your heart and lungs, supplying blood with oxygen and pumped back into the body, allowing your own organs to rest and to help the impaired function.” Why is ECMO important in saving lives?  “This state-of-the-art procedure can save lives in cases that otherwise would offer little chance of survival and can act as a bridge to those needing heart or lung transplants,” says Dr. Kimball Knackstedt, Cardiovascular surgeon, Renown Institute for Heart & Vascular Health, and Co-Medical Director of the ECMO program. “ECMO can be a game changer and a last resort for patients with severe heart and lung conditions. The procedure isn’t for everyone. ECMO requires anticoagulation, so a common complication is bleeding. This makes the therapy complex for trauma patients who often have life threatening heart and lung failure but due to severe bleeding, cannot utilize ECMO. This is truly a gift of life for many.”  ECMO programs growing across the U.S.  ECMO technology was developed in the late 1960s by a team led by Robert H. Bartlett, MD. Its main purpose was to serve as a long-term “bypass machine” to support the lungs or heart, which was not feasible at that time with conventional bypass machines. ECMO was first used successfully in 1971 by a patient with severe lung dysfunction after a motorcycle accident. In 1976, it was also used successfully for a baby who had severe lung injury from meconium aspiration, which led to increased use in the pediatric population. In 2009, ECMO was used worldwide in the treatment of severe lung failure during the H1N1 influenza pandemic. More than 100 new ECMO programs were launched in the U.S. in the last 3-5 years. Today, there are 6,120 hospitals in the U.S. and only 300 to 400 with ECMO programs according to the American Medical Association.  “We are launching our ECMO program with the capability to support up to 2 patients simultaneously for the first 1-2 years and aim to eventually perform 40 cases annually,” said Rob Blamire, Enterprise Project Manager for Renown. “Initially, we will focus on adult patients and later expand to include pediatric care. Depending on a patient’s needs, ECMO support varies from several days to weeks. Clinical researchers with the Healthy Nevada Project co-author research paper with findings that emphasize the need for a comprehensive approach to breast cancer risk assessment – including a focus on genetic medicine – to help ensure that individuals at high risk are identified and supported proactively rather than reactively.  Breast cancer is a leading cause of cancer death among women in the United States. According to the American Cancer Society, about 1 in 8 women will develop breast cancer and about 1 in 39 women will die from breast cancer. Breast cancer is associated with increased age, hereditary factors, obesity, and alcohol use. Since 1990, breast cancer death rates have declined progressively due to advancements in treatment and detection. In Nevada there are an estimated 2,310 new breast cancer cases a year, and genetic mutations such as in the genes BRCA1 or BRCA2 remain a top risk factor for this prevalent disease. Recognizing the urgency for progress in breast cancer research, a collaborative effort between physicians, advanced practice providers and scientists from the Healthy Nevada Project® (HNP) and Helix have unveiled groundbreaking research. This study explores how genetic screenings are a necessary supplement to traditional testing methods, together offering more accurate insights into a patient's likelihood of developing breast cancer in the future. HNP is operated by Renown Genomic Medicine and the Institute for Health Innovation and is one of the largest community-based population health studies in the country. Their team works in collaboration with Helix, a leader in precision health that delivers comprehensive genomic solutions. Together, this dynamic partnership aims to understand breast cancer risk factors and pave the way for more effective preventative measures. The combined research team studied 25,591 female HNP participants to evaluate the performance of different genetic screening approaches to identify women at high risk of breast cancer. The results of this research suggest that a combined monogenic, or single-gene, and polygenic, or multi-gene, approach to breast cancer screenings helped produce more accurate results and more closely identify study participants who have a high genetic risk of developing the disease. "Based on this research, we are advocating a shift in approach which would improve breast cancer risk assessment through a combination of effective family history ascertainment and genetic screening,” said Joseph Grzymski, PhD, principal investigator of the Healthy Nevada Project, research professor at the University of Nevada, Reno School of Medicine and co-author of the breast cancer research paper. “This tailored approach, founded on the assessment of individual genetic risk, not only intends to elevate patient well-being but also will improve efficiency and equity in healthcare." Complementing the team’s research on leveraging genetics to identify women at low genetic risk of breast cancer that could safely defer mammogram screenings by five to 10 years that was released in late 2023 in JAMA Oncology, the study suggests that incorporating genetic information can assist in personalizing breast cancer screenings and optimizing the use of screening resources. "Existing disparities persist across various facets of breast cancer screening and treatment; however, genetic screening is clearly a powerful tool to help facilitate early intervention for those at higher risk,” said Jamie Schnell Blitstein, APRN, a primary care nurse practitioner at Renown Health and co-author of the breast cancer research paper. “By placing a heightened focus on risk, we underscore the pivotal role of preventative breast cancer screening.” Despite the availability of effective methods for early screening, co-authors of this research found that 78 percent of women with a family history of breast cancer had their risk ascertained only after a breast cancer diagnosis. The findings emphasize the need for a comprehensive approach to breast cancer risk assessment – including a focus on genetic medicine – to help ensure that individuals at high risk are identified and supported proactively rather than reactively.  “These findings that can profoundly impact how healthcare is delivered were only made possible by all the participants who were willing to consent to research,” said Alex Bolze, PhD from Helix and co-author of the publication. “Broad-scale collaboration projects like these between Renown Health and UNR that engage large populations where participants share both their genetic information as well as electronic health records drive advancements in preventative medicine, as well as fundamental biological research.”   The research paper was officially accepted on Jan. 29, 2024, and will be published by Elsevier, Inc. on behalf of the American College of Medical Genetics and Genomics. Physician researchers seek to understand how the immune system responds to COVID-19 and create a healthier Nevada. During the early stages of the pandemic, convalescent plasma was considered the only viable treatment option available for patients with COVID-19. Convalescent plasma is the component of the blood from recovered patients that may contain COVID-19 antibodies that help fight the infection. The National Institutes of Health has since developed treatment guidelines for COVID-19 based on clinical trial data and many studies are still underway worldwide assessing various additional treatment options. Convalescent plasma was in high demand but difficult to locate for COVID-19 patients in the northern Nevada area. A 24-year-old nursing assistant, Austin Meegan, was hospitalized and spent weeks staving off kidney and lung failure before learning he was eligible for an experimental blood transfusion that showed promise in treating COVID-19. Doctors estimated Meegan had only about a 3% chance of tracking down a donor to match his rare blood type. A COVID-19 survivor, Thomas Gibson, a Texas resident with the same blood type, traveled to Reno to donate his viral antibodies and a convalescent plasma donation credited with helping to save Meegan’s life. Physician clinical researchers and scientists at Renown and the University of Nevada, Reno School of Medicine (UNR Med) knew they needed to create better options for patients and physicians. Clinical researchers developed a study to help other patients like Austin, and pleaded for donations from recovered COVID-19 patients to donate their convalescent plasma. The researcher teams looked to understand how the body’s immune system responds to the virus over time, to aid them in developing new treatments for COVID-19. “The world’s capacity to get through the COVID crisis will depend on four things — science, technology, innovation and partnerships, says Tony Slonim, MD, DrPH, President and CEO of Renown Health.  “Taking lab bench discoveries to the bedside of patients in an efficacious and timely manner is not easy, but with UNR Med and our partners, we are making great strides in advancing clinical research which has the power to save lives and to create a healthier Nevada.” “It’s tremendously promising to partner on clinical research that will not only help us better understand the disease, but help inform treatment for those combatting COVID-19 that has had such a devastating impact on Nevadans, our nation and the world,” says UNR Med Dean Thomas L. Because there may be limited supply, the vaccine continues to be distributed in a tiered approach. Administration of the final doses of the vaccine is an important pandemic milestone as it brings an overwhelming sense of hope and relief to all of us at Renown,” said Tony Slonim, MD, DrPH, President & CEO of Renown Health. “The healthcare heroes vaccinated today have been batting this pandemic on the front lines and working around the clock for the past 11 months to keep our community safe. Administered 21 days after the initial dose, this dose marks the final dose of the vaccination series. Many Renown employees have also received the Moderna vaccine, of which second doses will be administered after 28 days.  The intricacies of vaccine administration Renown’s COVID-19 vaccination process includes pre-scheduling physicians and employees through the Epic MyChart electronic medical record used by Renown. Employees are masked, and in their personal vehicles, as they drive through the vaccination site at Hometown Health. Each physician and healthcare worker provides their verification, identification, documentation and consent. They then extend their upper arm to receive the vaccination. Following the receipt of the vaccination, they have a 15-minute observation period to assure they do not experience any immediate side effects.  Similar to receiving the first dose, these Tier 1A healthcare employees were encouraged to schedule their vaccination after their completed work schedule to have time to rest. As with all vaccinations, there is a potential for injection site reactions (redness, swelling and pain) as well as fever, fatigue, headache, chills, vomiting, diarrhea, muscle pain and/or joint pain. Fever is not an infrequent side effect, especially on the second dose, so these vaccinated employees will self-monitor their temperatures, as they cannot work at Renown with a fever. Renown invited media to help commemorate this triumphant vaccination event. Beginning Thursday, January 14, 2021, in order to quickly vaccinate additional residents of Washoe County age 75 and older, Renown Health is pleased to partner with the Washoe County Health District to help distribute the initially available, limited supplies of COVID-19 vaccines.  Those who are 75 years or older interested in joining the waiting list for an appointment at Renown’s Drive Thru COVID19 vaccination clinic, will need to download Renown MyChart, if they currently do not have an account. Residents do not need to have a Renown Health provider to enroll. Residents will be notified through their MyChart account when appointments are available. The vaccine clinic is operating by appointment only. Due to limited resources and the limited vaccine available, initially the Renown COVID-19 Vaccination Program will concentrate on vaccinating those 75 and older at this time in Washoe County. Per Governor Sisolak, and the state’s vaccine rollout plan, once those 75 and older have the opportunity to receive the vaccine, the next general population priority group is those who are 70 and older. In addition, Renown Health patients and members of Senior Care Plus/Hometown Health aged 75 and older are will be notified through their Renown MyChart accounts as appointments become available. Once an appointment is confirmed, instructions are given on how to obtain the vaccine. “We remain steadfast in our determination to fight the good fight for our community. The tragedy of COVID-19 is that the mortality rates, or the risk of dying, is highest in this generation,” said Tony Slonim, MD, DrPH, President & CEO of Renown Health. “We are pleased to be part of the solution, to bring hope and to make a genuine difference in the health and well-being of others.”  Age increases risk for severe illness & hospitalization According to the CDC, the risk for severe illness with COVID-19 increases with age, with older adults at highest risk. The greatest risk for severe illness from COVID-19 is among those aged 85 or older. Severe illness means that a person with COVID-19 may require hospitalization, intensive care, or a ventilator to help them breathe, or they may even die. In Washoe County alone, there are 29,034 people 75 years or older and Renown is only receiving limited supplies of the vaccine. Older adults are at greater risk of requiring hospitalization or dying if diagnosed with COVID-19. As you get older, your risk of being hospitalized for COVID-19 increases. For those aged 75 and older, the risk of hospitalization is eight to 13x higher, with deaths 220-630 percent higher than for younger adults. Compounding the risk for COVID-19 for older adults is a weakened immune system. “Because the immune system generally weakens with age, it’s important that older adults get a vaccine to help their bodies form a defense against COVID-19” says Derek Beenfeldt, MD, Chief Medical Officer for Hometown Health, the insurance division of Renown. “Please get vaccinated, unless you are sick or have significant allergic reactions to vaccines."  You are advised not to receive the vaccine if: You are ill at the time of vaccination (fever, shortness of breath, vomiting, diarrhea or acute illness). Due to the unprecedented demand of local residents, Renown now has a full “waiting list” for those interested in the COVID-19 vaccine, and will be working through the list to offer appointments as they become available.  Renown continues to vaccinate physicians and its essential workforce for COVID-19, and to provide nasal swab testing for those with a lab order from their provider, for COVID-19 symptoms. In addition, Renown has stretched to support the Washoe County Health District to meet the needs of those aged 70 and older who are now eligible to receive a COVID-19 vaccine.  Over this past weekend, more than 3,000 of Washoe County’s oldest residents were able to obtain an appointment, and receive their first dose of the COVID-19 vaccine at Renown’s Drive Thru COVID-19 vaccination clinic. Renown posted 3,000 appointments for those on the MyChart electronic waiting list and 100% of appointments were booked within an hour. “We want to help the Health District, and we are working to vaccinate as many people as possible, given our limited staffing, volunteers, space and vaccine,” says Melodie Osborn, RN, Chief Nursing & Quality Officer for Health Services. “We are glad to step in and help; and our goal is to always provide an exceptional vaccine experience. Our team and our systems are definitely feeling the pressure as we see unprecedented demand for this vaccine. We ask the public to be patient with us, and to help support us in this important work.” Scheduling limited Both the Pfizer and Moderna COVID-19 vaccines require two doses, the second administered three to four weeks after the first. Renown is working to be sure those who receive their first dose also obtain their second dose in a timely manner. This includes 7,000 physicians and health care workers, and will now include those older adults vaccinated with their first dose. The complexity of the vaccine dosage requirements continues to draw down on the number of vaccines and appointments available.  Renown MyChart and phone systems strained under the demand Last Thursday, when Renown announced that those who are 75 years or older could join the waiting list by downloading Renown Epic MyChart, so many people logged in that the system was strained to accommodate the additional demand. Renown has now doubled the capacity of the Epic MyChart system to be sure all goes smoothly.  Renown Health has been able to distribute and administer the COVID-19 vaccine to a record number of people in the shortest possible time. Stage 2 Air Quality Index and alarming research studies from DRI, Renown Health and the Washoe County Health District leads to public health advisory- seek care if needed For people who suffer from asthma, wildfire smoke is more hazardous than other types of air pollution, according to a study from the Desert Research Institute (DRI), the Renown Institute for Health Innovation (Renown IHI) and the Washoe County Health District (WCHD). The study published in the journal Environmental Health examined associations between airborne particulate matter (PM) from sources such as wildfire, transportation and industry, and medical visits for asthma at Renown Health’s emergency departments and urgent care centers in Reno, Nev. during the six-year period from 2013-2018. In places like Reno, where wildfire events occur regularly during parts of the year and are expected to become more frequent in the future, an accurate understanding of the impacts of wildfire smoke on population health is critical. We have an exceptional process in place through our urgent care sites, Renown Medical Group offices and hospital emergency departments to screen, diagnose and treat members of our community who come to us for care related to asthma, chronic obstructive pulmonary disease (COPD), chronic bronchitis, emphysema, and other upper respiratory diseases that may be exacerbated by wildfire smoke. At the same time, we continue to develop and refine, in real-time, the data that supports our predictive analytic models. We are using every tool and resource-including scientific data and clinical reporting, to ensure that we are meeting both the health and healthcare needs of the people we serve. In the study, researchers found that air quality in the Reno area was affected by wildfire smoke on a total of 188 days during the study period. A total of 18,836 asthma-related emergency room and urgent care visits occurred over the same five-year period of time, indicating that the influences of wildfire smoke and other types of air pollution on this medical condition are important to understand. The Washoe County Health District – Air Quality Management Division (AQMD) has issued a Stage 2 Air Pollution Warning due to smoke from area wildfires. Air quality in the Reno-Sparks area is now “Very Unhealthy” with an Air Quality Index of 247. The Stage 2 warning means that all residents should stay indoors and reduce activity levels due to the susceptibility of increased health risks. To see current air quality in Reno-Sparks, click here. Brion Hill, MD who practices at the Renown Urgent Care - Los Altos location warns, “This is an important health advisory, exposure to elevated PM2.5 (fine particulate matter in smoke) concentrations can result in eye and throat irritation, headaches, nausea, shortness of breath, congestion, coughing, impaired lung function and chest pain. Everyone should take precautions when the air quality is unhealthy. Air pollution can aggravate heart and cardiovascular disease as well as lung diseases like asthma and COPD. When the air quality is unhealthy, people with these conditions may experience symptoms like chest pain, shortness of breath, wheezing, coughing, or fatigue.
